What is a Panic Attack?

A panic attack is a sudden surge of intense fear or discomfort that peaks within minutes. It can feel overwhelming and often occurs without a clear or immediate threat. While panic attacks are not physically dangerous, they can feel frightening and disorienting.

During a panic attack, the body’s “fight-or-flight” response is activated, even when there is no real danger present. This can lead to powerful physical sensations and distressing thoughts.

Panic attacks can happen occasionally or as part of a broader condition such as panic disorder. Many people experience at least one panic attack in their lifetime.

  • Panic attacks can come on unexpectedly or be triggered by specific situations

  • They usually peak within 10–20 minutes

  • Recurrent attacks may lead to fear of future episodes

Common Symptoms of Panic Attacks

Panic attacks involve a combination of physical, emotional, and cognitive symptoms.

Physical symptoms:

  • rapid or pounding heartbeat

  • shortness of breath or feeling smothered

  • chest pain or discomfort

  • dizziness or lightheadedness

  • sweating or chills

  • trembling or shaking

  • nausea or stomach discomfort

Emotional and cognitive symptoms:

  • intense fear or sense of impending doom

  • feeling detached from reality (derealization)

  • fear of losing control or “going crazy”

  • fear of dying

These symptoms can feel so intense that many people initially believe they are experiencing a medical emergency.

Panic-Related Disorders

Panic attacks can occur on their own or as part of different anxiety-related conditions.

Panic Disorder

Characterized by recurring, unexpected panic attacks and ongoing worry about having more attacks. This often leads to changes in behavior to avoid triggers.

Agoraphobia

Involves fear of situations where escape might be difficult or help unavailable during a panic attack (e.g., crowded places, public transport).

Other Anxiety Disorders

Panic attacks may also occur in social anxiety, specific phobias, or generalized anxiety.

How Do Panic Attacks Develop?

Panic attacks often follow a cycle:

  1. Trigger or sensation (e.g., slight increase in heart rate)

  2. Interpretation (“Something is wrong with me”)

  5. Reinforcement of fear

Over time, this cycle can lead to:

  • fear of having another attack

  • avoidance of certain places or situations

  • increased anxiety sensitivity

Sometimes, panic attacks seem to come “out of the blue,” which can make them feel even more distressing.

What Triggers Panic Attacks?

The exact cause varies, but several factors can contribute:

Biological factors

  • heightened sensitivity in the nervous system

Psychological factors

  • sensitivity to bodily sensations (e.g., noticing heartbeat changes)

Life experiences

  • major life transitions

  • trauma or prolonged stress

  • significant loss or change

Lifestyle influences

  • caffeine or stimulant use

  • lack of sleep

  • substance use or withdrawal
